Monthly Care Subscription

Service Description

For patients who benefit from continuity and follow-through over time, ongoing care is supported through a monthly subscription model. A session is up to 60 minutes and visits and communication with your doctor are unlimited throughout the month with this option. This type of membership session is eligible to book after your initial one-time on-boarding session (90 minutes). This structure exists to support care that happens between visits, not just during scheduled appointments. Subscription Care Includes: * Scheduled follow-up visits based on individual clinical needs * Ongoing interpretation of labs, symptoms, and response to interventions *Care plan refinement over time *Secure messaging for questions /clarifications between visits *Priority scheduling Subscription care is optional, it is not a long-term contract. This type of care is adjustable and may discontinue as needs change or concerns are addressed and managed. Whether subscription care is appropriate is discussed after the initial consultation, once your needs are clearly understood. **If you have not booked an initial on-boarding visit, you will not be able to book this type of session until the one-time on-boarding session is complete. **
