A bounded consultation model for patients whose appropriate evaluations have produced useful findings without a sufficient account of persistent multisystem symptoms

William Love, MD, FMCP-M | Board-Certified Internal Medicine | Love Functional Medicine, Mechanicsburg, Pennsylvania

Love Functional Medicine offers a bounded adjunctive consultation for selected Pennsylvania adults whose appropriate evaluations have not produced a sufficient account of persistent complex symptom patterns. The model preserves primary and specialty ownership, limits testing to decision-relevant questions, and returns findings to the existing care team with patient authorization.

The practice does not replace emergency care, longitudinal primary care, or disease-specific specialty management.

Patients most likely to fit

  • Persistent symptoms affecting function across more than one domain.

  • A completed or substantially completed initial evaluation without a sufficient explanatory account.

  • A need to synthesize chronology, prior testing, treatment response, sleep, medications, autonomic features, post-infectious patterns, and other clinical context.

  • Willingness to work within uncertainty and avoid indiscriminate testing.

  • Physical presence in Pennsylvania for clinical visits.

Patients who need another pathway first

  • Acute, unstable, or emergency symptoms.

  • An unresolved red flag requiring expedited specialty or hospital evaluation.

  • A request for confirmation of a predetermined diagnosis without reconsideration of alternatives.

  • A need for comprehensive primary care replacement, controlled-substance management, or a disease-specific procedure outside the practice scope.

What the consultation does

The consultation reviews the existing record, reconstructs the timeline, identifies what prior testing established, and develops a ranked account of the questions that remain. Additional tests are selected only when they can change probability or management. When findings belong to cardiology, neurology, gastroenterology, allergy/immunology, endocrinology, sleep medicine, psychiatry, or another discipline, disease-specific management remains with or returns to that clinician.

Examples of that reasoning applied to specific patterns: POTS evaluation, post-exertional malaise, and MCAS diagnostic criteria.

What the referring clinician should receive

A useful response should state the consultation question, relevant prior findings, current problem representation, working differential, any new findings, recommendations with rationale, medication or testing considerations, explicit ownership of follow-up, and conditions that warrant escalation. The patient should not become the sole courier of the clinical account.

What this model is not

It is not a full-panel screening service, a supplement protocol, a claim that every unexplained symptom has one hidden cause, or an argument that previous care was misguided. Negative evaluations are retained as evidence. Standard diagnostic and therapeutic care remains the floor.

How to begin

A referring clinician may discuss a potential fit before sending records. A formal referral is not required for a patient to inquire, but physician-to-physician communication is welcomed when the patient authorizes it. The immediate goal is one well-bounded consultation question — not transfer of the entire patient.

Questions referring clinicians commonly ask

Does Love Functional Medicine assume ongoing disease-specific management?

No. Disease-specific management remains with or returns to the appropriate primary physician or specialist.

What should accompany a referral?

The consultation question, relevant timeline, key results, current medications, active specialists, and the requested scope are more useful than an unfiltered chart alone.

Will the referring physician receive a consultation summary?

With patient authorization, the consultation returns a concise account of findings, recommendations, ownership, and escalation criteria.

This practice does not provide urgent or emergency care. Patients with acute or unstable symptoms should be directed to emergency services or expedited specialty evaluation.
